1) Information We Collect

  • Contact & Appointment Information: Name, phone number, email, mailing address, preferred contact method, appointment date/time, and related details you provide through forms, calls, texts, or emails.

  • Patient/Medical Information: Health history and treatment information you provide in connection with care. (See “HIPAA & Patient Information,” below.)

  • Technical Data: IP address, device and browser type, pages visited, and cookies or similar technologies to improve site performance and user experience.

  • Communications Data: Records of communications with us, including email and SMS/MMS messages, delivery status, and responses (e.g., STOP/HELP).


2) How We Use Information

  • To provide dental services, schedule and confirm appointments, and respond to inquiries.

  • To operate and improve our website, services, and patient communications.

  • To send administrative and appointment-related messages, including reminders and updates (email, phone, or SMS/MMS with your consent).

  • To comply with legal obligations and protect our rights and the security of our systems.

We do not sell your personal information, including your mobile number or SMS consent data. We do not share SMS opt-in/consent with third parties for their own marketing.


3) SMS/MMS Text Messaging Program (10DLC)

We may offer optional SMS/MMS messaging for appointment reminders, scheduling coordination, and important updates.

Enrollment & Consent

  • You will only receive texts if you opt in (e.g., checking a consent box on a form, texting a keyword, or providing written consent at intake).

  • Consent is not a condition of receiving treatment.

Message Frequency

  • Message frequency varies (e.g., a few messages per appointment cycle). We strive to send only necessary communications.

Cost

  • Message and data rates may apply based on your mobile plan.

Opt-Out

  • You can opt out at any time by replying STOP to any message. You will receive a final confirmation text and then no further messages, unless you re-opt in.

Help

Carriers & Delivery

  • Mobile carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier and network.

Privacy of Text Data

  • We use your mobile number solely to send the messages you consented to receive. We do not sell or share your number with third parties for their marketing. Service providers (e.g., messaging platforms) may process messages on our behalf under confidentiality agreements.

Eligibility

  • By enrolling, you confirm you are the account holder or have permission of the account holder and are 18 or older (or have parental/guardian consent where applicable).


4) HIPAA & Patient Information

We are committed to protecting your health information in accordance with applicable laws. We will provide (or have provided) our HIPAA Notice of Privacy Practices, which governs how we use and disclose protected health information (PHI) for treatment, payment, and healthcare operations.
Important: Standard SMS is not a secure channel for PHI. Please avoid sending sensitive health information via text. If sensitive details are needed, we will use or offer a more secure method.


5) Cookies & Tracking

We may use cookies and similar technologies to remember preferences, analyze site usage, and improve performance. You can adjust your browser settings to block or delete cookies; some site features may not function properly without them.


6) Sharing of Information

We may share information with:

  • Service Providers/Business Associates that help us operate (hosting, analytics, scheduling, messaging, payment, IT/security). They may only use information as instructed by us.

We do not allow service providers to use SMS consent data for their own marketing.

We retain information as long as needed for the purposes described, to comply with legal/record-keeping requirements, and to resolve disputes. SMS consent logs and opt-out records are retained as required by carrier and industry guidelines.


9) Your Choices & Rights

  • SMS/Email Preferences: Opt out of texts by replying STOP; opt out of marketing emails via the unsubscribe link.

  • Access/Update: Contact us to request access to or correction of your information.

  • Do Not Track: Our site may not respond to “Do Not Track” signals.

  • State/International Rights: Depending on your location, you may have additional rights (e.g., to know, delete, or limit certain uses). Contact us to exercise these rights, and we will comply as applicable.


10) Children’s Privacy

Our services are intended for general audiences and not directed to children under 13. We do not knowingly collect personal information from children under 13 without appropriate parental consent.
